Eligibility Requirements
The Primary Care Clinic at the Urban Inter-Tribal Center of Texas serves enrolled members of Federally or State recognized American Indian tribes and Alaska Natives.
Adult New Patient Appointment
To establish a chart at the Urban Inter-Tribal Center of Texas you must first fill out the new patient registration forms in the link below. You must include the required documentation to establish a new patient chart. Our Medical Records staff will reach out to you to schedule your initial appointment using the contact information you provide in the patient registration form.
New Patient Appointments must be completed before additional services can be rendered.
Required Documents - Adults
- Certificate of Degree of Indian Blood (CDIB) Card or Tribal Enrollment Document
- Driver's License or State ID Card
- Health Insurance Cards (Insurance information must be provided for those with existing coverage)
- List of current medications
Child New Patient Appointment
Parents must come into the Primary Care Clinic to register their child. Anyone that is not a parent must provide legal documentation that they have custody, guardianship, or foster care.
Required Documents - Children
- Certificate of Degree of Indian Blood (CDIB) Card or Tribal Enrollment Document (May use parent’s CDIB card until their 19th birthday)
- Certified birth certificate
- Social Security Card
- Immunization records
- Parent's Driver's License or State ID Card
- Health Insurance Cards (Insurance information must be provided for those with existing coverage)
- List of current medications (prescription and over-the-counter)
